The Ashworth College Transcript Request Form is located in the back of the Ashworth College Student Handbook, but you can also download the attached Ashworth College Transcript Request Form right here within the Community.

The attached Transcript Request Form is for all students currently enrolled in an Ashworth College degree program.  Degree programs include Associate, Bachelor and/or Master degree programs.  Please completely fill out and send the form to your high school or the state office that granted your GED certificate.  The college, high school, or GED office may charge a minor fee for sending your transcript to Ashworth.  Please send any fees along with the Transcript Request Form directly to the given institution and not to Ashworth College.  If you have a maiden name, please be sure this information is included on your Ashworth account.

It's very important that you submit your transcripts as soon as possible because we're unable to set up your program of study, provide you with credits for courses you've already taken, and send out your next course until your transcripts have been received and evaluated.
